What you will need to apply:
1. Proof that you live in Napa County
Such as: a Utility bill, rental agreement, or a personal check with your address.
2. Proof of income - one of the following:
Copy of recent pay stub or last year's federal income tax return, or written statement from employer stating gross monthly income and when it was earned, OR
Persons paid in cash may complete a form stating income and when it was earned, OR
Self-employed may use the previous year's federal income tax return with Schedule C.
3. Other Information
If your child was born in the United States, you will need a birth certificate and Social Security Card for the child.
If your child was NOT born in the United States, you do not need other documents.
